Internal Medicine: Forms

There may be a time when you need your doctor to complete a form for your employer, for disability, school/college or many other reasons. Here are some helpful tips to ensure the process is as smooth and as quick as possible.

When you drop off your form to be completed staff will ask you to complete and sign a required Release of Information which outlines who the form will be sent to as well as what specific information from your medical record that you are authorizing us to release. Without this release we cannot complete your form. Feel free to print the form out ahead of time using the link above and bring it to the office with your form. To speed up the forms process please be sure all of the sections to be completed by you are done when you arrive.

In most cases forms are complete and ready for pick up or mailing  in 7-10 business days. 

Verbal Release of Information forms can be completed by you which will allow your care team to speak to family members or other designated people regarding your appointments, test results and other medical information.  Verbal release is not all inclusive and some information may not be shared as outlined by you or your provider.
